TERHADAP Staphylococcus aureus DAN Escherichia coli

Manusia hidup di alam selalu kontak dengan mikroorganisme, bakteri, virus, fungi, dan berbagai bentuk kehidupan parasit. Infeksi terjadi bila mikroorganisme yang masuk ke dalam tubuh menyebabkan berbagai gangguan fisiologi normal tubuh sehingga timbul penyakit infeksi. Penyakit infeksi mempunyai kemampuan menular pada orang lain yang sehat sehingga populasi penderita dapat meluas (Wattimena dkk., 1991).

Infeksi merupakan penyakit yang sering terjadi di daerah tropis seperti Indonesia karena keadaan udara yang banyak berdebu, temperatur yang hangat dan lembab sehingga mikroba dapat tumbuh subur. Bagi negara berkembang timbulnya strain bakteri yang resisten terhadap antibiotik pada penyakit infeksi merupakan masalah penting. Kekebalan bakteri terhadap antibiotik menyebabkan angka kematian semakin meningkat. Penanganan penyakit infeksi tersebut tidak hanya meningkatkan biaya kesehatan karena diperlukan penanganan kombinasi antibiotik, tetapi juga menyebabkan meningkatnya kematian terutama di negara berkembang karena antibiotik yang diperlukan tidak tersedia (Heymann, 1996).

Penyakit infeksi masih menduduki urutan pertama dalam hal penyebarannya di Negara Indonesia, sehingga dibutuhkan biaya penanggulangan yang relatif besar terutama untuk obat-obat golongan antibiotik. Dana yang harus dikeluarkan oleh pemerintah Indonesia untuk mengimpor bahan baku antibiotik setiap tahunnya berkisar antara Rp. 18,6 – Rp. 122,4 milyar (Akmal, 1996).

Staphylococcus aureus merupakan penyebab infeksi piogenik (menghasilkan pus) pada manusia dan paling sering terjadi. Strain Staphylococcus aureus yang ditemukan di rumah sakit mungkin resisten terhadap penisilin karena mampu menghasilkan penisilinase yang merusak penisilin dan dapat menjadi resisten terhadap obat-obat lain. Sebagai penyebab infeksi stafilokokus “rumah sakit” Staphylococcus aureus bertanggung jawab atas separo kasus sepsis pada luka bedah, pada unit kebidanan menyebabkan abses payudara pada ibu-ibu, mata lengket dan lesi-lesi kulit pada bayi, ratusan kematian setiap tahun di Inggris karena infeksi stafilokokus yang terjadi di rumah sakit. Penyebaran Staphylococcus aureus ke dalam udara biasanya sering terjadi. Laki-laki lebih sering menyebarkan bakteri ini daripada wanita dan kulit di daerah perineum, merupakan sumber utama, sedangkan Escherichia coli merupakan flora normal di dalam usus. Escherichia coli dapat menyebabkan infeksi saluran kencing yang merupakan infeksi terbanyak (80%), gastroenteritis, dan meningitis pada bayi, peritonitis, infeksi luka, kolesistitis, syok bakteremia karena masuknya organisme ke dalam darah dari uretra, kateterisasi atau sistoskopi atau dari daerah sepsis pada abdomen atau pelvis (Gibson, 1996).
Minyak atsiri adalah suatu substansi alami yang telah dikenal memiliki aktivitas sebagai antibakteri. Bahkan minyak atsiri cengkeh telah digunakan sejak lama di berbagai rumah sakit Eropa untuk mengatasi infeksi Mycobacterium tuberculosis (Yulliasri dkk., 2000).

Minyak atsiri dapat menghambat beberapa jenis bakteri merugikan seperti Escherichia coli, Salmonella sp, Staphylococcus aureus, Klebsiella, dan Pasteurella (Agusta, 2000). Jeruk keprok (Citrus nobilis Lour.) merupakan salah satu tumbuhan yang menghasilkan minyak atsiri. Tanaman ini dilaporkan bahwa daun, akar, dan kulit buahnya mengandung alkaloid dan polisakarida. Disamping itu daun dan akarnya juga mengandung flavonoid, sedangkan daun dan kulit buahnya mengandung minyak atsiri serta akar dan kulit
buahnya mengandung saponin (Hutapea, 1993). Berdasarkan penelitian yang pernah dilakukan, minyak atsiri yang diperoleh dari hasil penyulingan daun jeruk nipis (Citrus aurantifolia Swingle.) memiliki aktivitas antibakteri terhadap Staphylococcus aureus dan Escherichia coli (Normasani, 2007). Sejauh ini belum ditemukan literatur yang melaporkan tentang aktivitas antibakteri dari minyak atsiri daun jeruk keprok, maka akan dilakukan penelitian dengan judul “Uji Aktivitas Antibakteri Minyak Atsiri Daun Jeruk Keprok (Citrusnobilis Lour.) terhadap Staphylococcus aureus dan Escherichia coli“.

B. Rumusan Masalah
Rumusan masalah dari penelitian ini adalah :
1. Apakah minyak atsiri daun jeruk keprok (Citrus nobilis Lour.) mempunyai aktivitas antibakteri terhadap Staphylococcus aureus dan Escherichia coli dan berapa Kadar Hambat Minimal (KHM) dan Kadar Bunuh Minimal (KBM) nya?
2. Komponen minyak atsiri apa yang mempunyai aktivitas antibakteri terhadap Staphylococcus aureus dan Escherichia coli?

Daun jambu biji tua mengandung berbagai macam komponen seperti kuersetin (flavonoid) yang berkhasiat untuk mengatasi penyakit demam berdarah dengue (DBD). Penelitian yang pernah dilakukan oleh Soegeng Soegijanto, Bagian Ilmu Kesehatan Fakultas Kedokteran Universitas Airlangga (UNAIR) Surabaya pada tahun 2004 menunjukkan bahwa, ekstrak kental daun jambu biji bisa menghambat pertumbuhan virus dengue penyebab DBD dan meningkatkan jumlah trombosit hingga 100 ribu milimeter per kubik tanpa efek samping. Pada uji keamanan (toksisitas) ekstrak daun jambu biji termasuk zat yang praktis tidak toksik (Anonima, 2008).
Seperti diketahui, DBD merupakan penyakit yang disebabkan oleh virus dengue dengan angka kematian dan kesakitan yang cukup tinggi. Sampai saat ini pengobatan DBD masih bersifat suportif, yaitu mengatasi kehilangan cairan plasma dengan cara memberikan minum 1,5 liter dalam 24 jam (air teh, gula, atau cairan lain) serta pemberian kristal diare, yaitu garam elektrolit (oralit). Untuk
pengobatan kuratif secara formal masih terbatas (Bermawie, 2006).
